Treatments

Advanced care, matched to the problem

Many of our patients have already tried the usual treatments. We look for the reason a wound or deformity isn't healing, then fix that reason.

Diabetic & complex wounds

Ulcers, pressure injuries, and surgical wounds that won't close. Debridement, offloading, advanced dressings, and a plan that addresses blood flow and infection.

For: sores open four weeks or longer, any wound with diabetes

Limb preservation surgery

Surgery for deep infection and bone infection (osteomyelitis), with staged procedures that save as much of the foot as possible.

For: patients told they may need an amputation

Charcot reconstruction

Early casting and protection for the warm, swollen Charcot foot, and surgical realignment when the foot has collapsed.

For: neuropathy with a swollen foot or changing foot shape

Muscle flaps & skin grafts

Orthoplastic reconstruction that brings healthy tissue to cover exposed bone, tendon, or hardware so a wound can close for good.

For: large or deep wounds, exposed structures

Microsurgical nerve surgery

Decompression and repair of injured or trapped nerves in the foot and leg, for selected patients with nerve pain, numbness, or neuroma.

For: burning, tingling, or shooting pain after injury or surgery

Reconstructive foot & ankle

Correction of deformities, unstable joints, and pressure points that cause pain or keep wounds coming back.

For: collapsing arch, ankle instability, toe deformity

Complex problems deserve an unhurried first visit. Here is how care at Evolve Podiatry usually unfolds.

    Records & referral

    Your doctor faxes a referral, or you call us directly. We gather imaging, labs, and prior notes before you arrive.

    Full evaluation

    Dr. Weiland examines circulation, sensation, the wound or deformity, and your shoes. Imaging or vascular testing is ordered if needed.

  3. 3

    A clear plan

    You leave knowing the cause of the problem, your options, and the next step, with coordination to your other doctors.

  4. 4

    Treatment & follow-through

    In-office care or surgery, then close follow-up, protective footwear, and prevention so the problem doesn't return.

The two-minute daily foot check

When feeling in the feet is reduced, small injuries go unnoticed. A quick look every day is one of the most effective ways to protect your feet.

Call us the same day if you see

A new sore or blister, drainage on your sock, a spot that is red, warm, or swollen, or any change in the shape of your foot.

  1. Look at the tops and bottomsUse a mirror or ask someone to help you see the soles and heels.
  2. Check between the toesLook for cracks, moisture, or skin breakdown.
  3. Feel for warm spotsCompare one foot to the other with the back of your hand.
  4. Look for changesCuts, blisters, calluses with dark spots, color changes, or swelling.
  5. Check your shoes before putting them onFeel inside for stones, seams, or anything that could rub.
  6. Moisturize, but not between the toesDry skin cracks; moisture between toes invites infection.

Referrals welcome

Send us patients whose lower-extremity problems need a surgeon's eye. Earlier referral keeps more options open.

  • Wound not clearly improving after four weeks
  • Wound that probes to bone, or exposed tendon or hardware
  • Warm, swollen foot with neuropathy
  • Dry gangrene or darkening toes
  • Recurring ulcers or deformity causing pressure
  • Nerve injury, entrapment, or neuroma

Please include demographics, insurance, recent notes, imaging, vascular studies, and labs. Spreading infection with systemic signs belongs in the emergency department first; call us so we can coordinate.

Dr. Jeffrey Weiland

Dr. Weiland is a podiatric surgeon in Pensacola who focuses on the hardest problems in the foot and ankle: wounds that won't heal, feet threatened by infection or Charcot collapse, and nerve injuries that limit daily life. He combines limb preservation surgery, soft-tissue reconstruction, and microsurgical nerve work, and he coordinates closely with each patient's other doctors. He also founded ILĒP, the Institute for Lower Extremity Preservation, to train nurses in wound and limb care.

Do I need a referral?

Referrals are welcome, and many patients come to us through their doctor, a wound center, or a hospital. Some insurance plans require a referral. Call us and we'll help you check.

What is the Friday open clinic?

Open clinic runs Fridays from 2:00 to 4:30 PM. All other visits are by appointment. Please call ahead if you can, so we can have your records ready.

Will I need surgery?

Not always. Many wounds and foot problems improve with the right offloading, wound care, and treatment of the underlying cause. When surgery is the better option, Dr. Weiland will explain why and what recovery looks like.

I was told I need an amputation. Can you help?

A limb preservation evaluation can show whether other options exist. Bring your imaging, recent labs, and any vascular studies so the evaluation is as complete as possible.

Do you accept my insurance?

Call the office with your plan information and we'll verify your coverage before your visit.

How is Evolve Podiatry related to ILĒP?

Evolve Podiatry is Dr. Weiland's clinical practice, where patients are seen and treated. ILĒP, the Institute for Lower Extremity Preservation, is his limb preservation and nurse education program.
